Sorry, I saw this thread multiple times but never clicked on it because I don't know much about N64. To me it looks like an I/O board for controller inputs on a N64 which you've obviously figured out. From there it could have been used for any purpose like testing a game with a pre-programmed set of controller inputs sent from a computer or it could be something more recent, used for tool assisted speedruns. I know this guy says it's from a kiosk but he didn't cite and resources so I can't just believe someone.

that sounds like one of those kitchen accessories nobody needs but someone still makes because it looks good in a large kitchen, and you think you need it when you don't ?

maybe posting on a japanese N64 collector's group will help?
