Sorry, I saw this thread multiple times but never clicked on it because I don't know much about N64. To me it looks like an I/O board for controller inputs on a N64 which you've obviously figured out. From there it could have been used for any purpose like testing a game with a pre-programmed set of controller inputs sent from a computer or it could be something more recent, used for tool assisted speedruns.
