Would be a high quality reproduction ( down to even adding some flaws to the box) but something about this looks "off" to me.

Never seen that, but it wasn't unlikely for stores to add them. A few of my Game Boy games have them, and I'm pretty sure that wasn't from the factory.

Stores usually don't have any issues defacing game boxes  I've seen them do worse.

Not saying that's what happened here, I'm sure someone else is more knowledgeable, but I do think it's a lot more likely than it being a bootleg. If it's a boot, wouldn't they be interested in making it look more authentic?

Here's confirmation from@ThePhleo that Trojan existed in a hangtab game pak.

Looks legit to me. Above average condition and hangtab is unpunched, but otherwise looks appropriate. And as others have said, some Capcom games did have hangtabs.
