Someone posted this in a Facebook group and I got his permission to repost here. Anyone seen this before? It’s inside the Y-fold seal:

The person who got this said they could only find one mention of it from 2004 from a serious collector:

My sealed copy has that as well, it's under the wrap.  I bought it years ago on amazon (I think .ca, but maybe .com?  I could check if you want me to).  I think i bought it in 2013.  My memory of it was that it wasn't super hard to find at the time, but maybe that was because noone cared?  Frankly, back then most gamecube games were super easy to get your hands on if you cared.

I’d love to know more so yes let me know if it was .com or .ca!

Dang, must've been an ebay purchase as it doesn't appear in either amazon.ca or .com orders.
