Consumer Precautions booklets started in '92 as far as I can tell. The earliest ones I can find all have a copyright date of '92 - so that's my best guess.
There are tons of games with registration cards. Atlus and Konami (then Ultra) games almost always had them to name a few.

MAGFest is a great con, but as someone who has been going for many years now I would say it's no longer top tier for collectors. There are usually only 2-4 booths for retro games and they are mobbed within a few hours of the vendor hall opening. That said, it's an amazing con for retro gamers with the arcade, the console hall, events, etc. I would say go to play games, not to find your grail

Hey @Goodvibes - here's pics from my personal collection. I'm not really into the TMNT games but it has the rest. If there's others you're looking for let me know. I'm pretty big into CIB Gameboy, Gameboy Color, and Gameboy Advance and I always strive to get all the inserts.

@RH I've noticed the same thing. The prices in my collection haven't updated in at least 2 weeks.
