DoctorEncore | 3,689
I updated the OP with the lists of games provided thus far. I'll have to go back and put them in alphabetical order, but it's a start.

All of these do have the RPTV warning has well. Overlord Ultima V: Warriors of Destiny Rollerblade Racer Alien3 Mickey's Safari in Letterland Zen the Intergalactic Ninja Yoshi's Cookie Kirby's Adventure I don't see any patterns in these games. Yoshi's Cookie and Kirby's Adventure are both first party too. Do these manuals have the full-page, front and rear projection warning? Or just the brief statement about rear projection? The rest are a mix for the format and usually follow the publishers' style. Kirby and Yoshi have the full page with red headings, Mickey and Rollerblade have a simpler full page warning, Zen puts it's after the warranty and epilepsy warning, and Alien 3 puts it in a yellow box in the corner of the first page. So maybe Nintendo mandated it at this point, but didn't have a required format. Also was looking at Kirby and noticed the game title has a TM* on the manual. I'm not sure if I've seen this on any other manuals, but that may be another mystery to add to the list.

Big thanks to @0xDEAFC0DE for reviewing hundreds of manuals to contribute to the list below. Sorry, I wasn't clear enough on this. Nothing July 1993 and on (except Color a Dinosaur and The Addams Family: Pugsley's Scavenger Hunt which were close to the transition) has either the Circle-T or the full projection warning. The list I gave with no Circle-T and projection warnings was from January 1993 - May 1993. Also I only checked games released 1993 or later, which is only 59 games not hundreds. I'll probably go through more when I have the free time though. 1 hour ago, DoctorEncore said: So maybe Nintendo mandated it at this point, but didn't have a required format. [Picture of Kirby's Adventure Manual] I've seen the TM* on a couple of different manuals. The * is just a footnote referring to the copyright information at the very bottom of page 3. Here's a picture for reference.

DoctorEncore | 3,689
Just checked out some more manuals. The star is definitely supposed to indicate the presence of the brief rear projection warning. I'm guessing the games with a star and Circle-T on the manual were just errors. 0xDEAFC0DE | 1,277
Ok, I went through all the manuals at the three sites below as well as a lot of ebay searching to figure out what manuals came with what markings. EDIT: I also went through everything posted in this thread. https://www.gamingalexandria.com/highquality/NES/http://www.digitpress.com/library/manuals/nes/https://www.nesfiles.com/Games Pretty quickly, I realized that the difference between the star code and the circle-T is not the format but the actual text used. The star code warning reads: Quote Nintendo recommends against using a rear projection television with you NES as image retention on the screen may occur. The circle-T warning reads: Quote WARNING: DO NOT USE WITH FRONT OR REAR PROJECTION TV Do not use a front or rear projection television with your Nintendo Entertainment System® (''NES'') and NES games. Your projection television screen may be permanently damaged if video games with stationary scenes or patterns are played on your projection television. Similar damage may occur if you place a video game on hold or pause. If you use your projection television with NES games, Nintendo will not be liable for any damage. This situation is not caused by a defect in the NES or NES games; other fixed or repetitive images may cause similar damage to a projection television. Please contact your TV manufacturer for further information. I only found a handful of manuals that didn't quite follow this. 17 manuals had the Circle-T style warning but did not any markings on the cover. Castle of Dragon has a star code style warning with no markings on the cover. As we've already discovered NES Play Action Football as well as Paperboy and Mission: Impossible all have both the star and circle-T but contain the circle-T style warning. Mega Man contains only the circle-T marking, but has both warnings. Times of Lore has the circle-T marking, but no warning (scan could be missing a page but it looks complete). I also noticed another trend. The switch to the star manual seemed to happen right after the switch to the Oval R SOQ. I only found a handful of games pre-1991 with the Oval R SOQ and no warnings. Meanwhile, Zelda 1 is the only game I fount that has a star code and an Oval TM SOQ.Finally, here's the list of what markings and warnings every NES game came with. It is by no means complete but has at least one manual variant for every game (well Sword Master I could only find a scan for the UKV version and a couple more I couldn't find a complete scan). When possible, I link to a scan. To explain the acronyms, NM/W = No Marking or Waring, CT = Circle-T, Other is all the exceptions I described above with the format xMyW (x Marking with y Warning, S = star, T = circle-T, N = no, D = double), Conf means I couldn't find a scan but confirm it exists, NC = not confirmed which usually means I found a picture of the cover but an absent or incomplete scan prevented me from finding the warning. Sure, here's the Google sheets link: https://docs.google.com/spreadsheets/d/1q5FEXvZG2KJP8TAMHmdzHUGbZmyPZ6ItCtQdIqqSreM/edit?usp=drivesdk 1 hour ago, DoctorEncore said: @0xDEAFC0DE: Also, can you drop that table behind a spoiler tag to make this page more easily navigable? I meant to do that but forgot. I don't have access to my computer tonight and it's impossible to do on mobile, so I'll do it tomorrow morning.

Yeah, they also have it listed as such in their black box guide. In one of the threads linked in the first post (https://connect.gocollect.com/discussion/comment/3097039#Comment_3097039) @K.Thrower (Wata's chief grader) gave some evidence as to why Wata thinks they were promotional. But, later the updated "For sale in..." text was brought up and it seemed like he wasn't aware of that theory. From all the evidence I've seen, I still think the box star code denotes the the updated "For sale in..." text. We haven't yet found an exception, it would be somewhat consistent with the manual's star code, and it would explain why it's reasonably common. I would be interested if Kenneth has any additional insight though.

So unless they actually publish some proof, I'm thinking it just signifies an update to sales/warranty. This is in line with the star code on the manual which signifies an update to include the brief rear projection warning.
Speedy_NES | 102
I'm assuming that copy was graded by Wata before I pointed out the alternate theory in the black box guide thread. If not, then it just seems like a way to better market star code boxes. But really, there is zero evidence that the star boxes are related to promotions. This was all based on an early thread on NA where someone mentioned his or her Dragon Warrior (IIRC) promotional copy having a star box. In the same thread, someone mentioned receiving the same game with non-star box through the same promotion. Additionally, there are (much rarer) "Nintendo Promotional Offer -- Not For Resale"-stickered boxes of games and accessories, all limited to first-party items. Those latter ones are much more likely to be actual promo versions from giveaways and such, not the star boxes, which clearly align with a box update.

Regarding the NFR stickers, I know they were at least used when NOA employee's bought games (to discourage reselling). I got confirmation from Archon 1981 at the bottom of this thread (https://web.archive.org/web/20191111060700/http://74.86.217.179/forum/messageview.cfm?catid=5&threadid=188976). He had a Dr. Mario that he bought directly from an employee. Unfortunately, the picture he attached wasn't archived, but I remember it having both the "Nintendo Promotional Offer -- Not For Resale" sticker and the "NOA Employee sale" sticker that's on the Tetris in the first post. For reference, here's a picture of the two Zelda's with the NFR sticker (http://web.archive.org/web/20090725071306/http://zeldacollectorsmuseum.com:80/). Of course they were also likely used for promos as well. In this thread (https://web.archive.org/web/20191111061719/http://74.86.217.179/forum/messageview.cfm?catid=10&threadid=109939) limbofunk mentioned, "most of them were Nintendo Power and prizes at Nintendo sponsored events, but those labels were also used in some instances for the NoA Library to be 'rented out' by employees."
